First ask the receiving service which documents and translation format it needs. Insurance, legal and immigration requirements may differ from clinical requirements.
Select records relevant to the appointment
Start with the latest discharge summary, operations, allergies, important diagnoses, current prescriptions and relevant investigations. Prepare a short dated chronology in English. Retain originals alongside translations.
For medicines, include the international active ingredient, strength and formulation, not only brand names. Laboratory results need units and reference ranges; imaging may require the actual image files rather than just the report.
Establish who arranges translation
Ask whether professional translation is required and whether the NHS service can arrange translation of clinically necessary records. If commissioning it yourself, obtain requirements for language, translator qualifications, certification and format first. Agree scope and price in writing, including unclear abbreviations and later corrections.
Request an interpreter for the conversation

Do not make a child responsible for interpreting complex consent or sensitive history. If bringing a relative, also discuss professional interpretation and confidentiality.
Check delivery and the next plan
Use the transmission channel specified by the practice or clinic. Avoid uploading identity documents and complete medical histories to unknown translation services. Automated translation may help draft a list, but errors involving doses, negatives and diagnoses need checking.
At the appointment, confirm that key records arrived. Overseas results do not always replace further investigation: methods, timing and the clinical question may differ. Ask what has been accepted, what needs clarification and who will arrange the next step.
