A form may ask about treatment, NHS treatment, unpaid bills or the purpose of a future visit. These are different questions. Do not answer no merely because care was free, an insurer paid or you consider the appointment minor.
Record the factual dates, institution, nature of the attendance and funding to the extent requested. Do not copy another person’s community answer. If the question’s meaning is unclear in your circumstances, retain its wording and obtain advice before submission.
Reconstruct the evidence
Check clinic letters, appointment confirmations, statements, invoices and bank payments. Request missing dates or amounts from the institution rather than inventing them. Where explanations are possible, identify approximate details and why they are approximate.
Distinguish IHS payments, personally funded treatment and insurer payments: these are not the same transaction. Evidence should explain the actual attendance. Avoid supplying an entire sensitive medical history where a specific requested confirmation is sufficient, while following the personal UKVI checklist.
Check outstanding NHS charges
If an NHS bill was issued, ask the institution’s billing or overseas-visitor team to confirm its status. Match the invoice reference, amount, payment and remaining balance. The Home Office publishes NHS cost-recovery guidance ↗. Not being asked to pay at reception does not establish that an issued bill is settled.
Under Part Suitability SUI 16.1 ↗, certain notified unpaid NHS overseas-visitor charges totalling at least £500 may be a refusal ground. This does not mean every past treatment or NHS appointment leads to refusal. Obtain specific advice about disputed bills or debt, and keep evidence of resolution.
If the next visit is for treatment
Separate previous treatment history from evidence of the new purpose. A visitor trip for private medical treatment ↗ requires evidence of arrangements, estimated costs, duration and funding. GOV.UK specifies a medical letter covering relevant information.
Prepare a realistic treatment and accommodation budget and check the appropriate permission length. A visitor visa is not a promise of free care. Before submission, reconcile past-treatment answers with evidence and the future purpose. If you discover a material error after applying, notify UKVI through the official channel and keep the explanation and confirmation.
